More Than Primary Care—Embedded Specialty Programs
The Extensivist Clinic
Leaving the hospital is just the beginning of recovery. The Extensivist Clinic makes sure you have somewhere to go next.
If you've recently been discharged and don't have an established provider, the Extensivist Clinic will see you within one to fourteen days of leaving the hospital. From there, you'll be connected with a family or internal medicine provider for continued care at our Post Falls clinic. Scheduling is coordinated through your hospital discharge team.

If you are interested in one of our embedded specialty programs—including behavioral health, memory care, addiction medicine, psychiatry, or sports medicine—a referral from your internal medicine provider is required.
