How to Get Your Records

Access Your Records in MyChart (Fastest Option)

  1. Open MyChart.
  2. Sign in or create an account.
  3. View or download your health information.

Go to MyChart

Request Records Online

  1. Complete the online request form.
  2. Upload a photo of your driver’s license (phone or webcam).
  3. Submit your request.

We’ll review your information and process your request.

Paper Records Request

  1. Download and complete the authorization form.
  2. Send it to Medical Records by mail, fax, or email. View contact details.
  3. We’ll verify your identity before releasing records.
  4. Choose how you’d like to receive them:

    ◦ Secure email
    ◦ Mail
    ◦ In-person pickup
    ◦ Sent directly to another provider

Note: Large files may be sent in multiple emails.
