Discharge Expectations
Discharge can take time as medications, instructions, transportation, and follow-up needs are finalized. Your care team will review what to expect before your loved one leaves the hospital.

Many patients need follow-up care after leaving the hospital. Help schedule appointments, track dates, and make sure questions are ready for the next visit.
When to Call (Red Flags)
Call the care team or seek medical attention if symptoms worsen, new symptoms appear, medications cause concerns, or recovery is not going as expected. Specific instructions will be provided at discharge.
At-Home Support Resources
Depending on your loved one’s needs, support may include home health, rehabilitation, medical equipment, nutrition support, or community resources.
Community and Patient Support Services
Families can schedule consultations regarding services that may be needed for an elderly parent or disabled loved one such as guardianship issues, resources, or end-of-life decisions.
