What to Expect at Your Visit
What to Bring
Please bring the following items to every visit so we can verify your information is current and accurate for billing purposes.
- Photo ID
- Current insurance card
- Secondary insurance card (if applicable)
- Subscriber date of birth and mailing address if you are not the policy holder
- Payment method for co-pay or balance due
At each visit, we’ll ask you to verify your insurance and contact information to help ensure claims are processed accurately and billed in a timely manner.
Payment at the Time of Service
Most insurance plans require a co-pay at the time of your visit. Depending on your coverage, you may also be responsible for deductibles, coinsurance, or services not covered by your plan.
We’ll submit claims to your insurance when applicable, including secondary insurance coverage when provided.

After Your Visit
After your insurance processes your claim, you may receive a statement for any remaining balance, including deductibles, coinsurance, co-pays, or services not covered by your plan.
Please review both your insurance Explanation of Benefits (EOB) and your billing statement carefully.
If payment is not received from your insurance company within a reasonable amount of time, the remaining balance may become your responsibility.
If an overpayment occurs, refunds are issued after insurance processing is complete.
