Once you’re connected with our specialty clinics, a nurse navigator becomes your direct point of contact. Instead of calling into a general line, you have someone who knows your history and can help in real time—whether that’s answering questions, walking you through injection training, reviewing labs, or helping adjust medications when something isn’t working. This approach earned national recognition through the Lighthouse Award, helping patients manage their condition more consistently and avoid unnecessary emergency visits.

Refer a patient with IBD or liver disease

Your patient will be contacted by a triage nurse within a day or two, with most appointments happening usually within a few weeks. We prioritize same day appointments if symptoms warrant.
