Refer a Patient to Kootenai Infusion Services

Kootenai Infusion Services works with community providers to make the referral, authorization and scheduling process as straightforward as possible. Follow the steps below based on where you practice to refer a patient for infusion services.

For Kootenai Providers

Please enter the appropriate Therapy Plan or Complex Care Plan in Epic for the treatment you are requesting for your patient. Once the plan is completed, a referral will automatically be generated and sent to the authorization team for review.

After prior authorization has been obtained, Kootenai Infusion Services will contact the patient to schedule their appointment.

If you need assistance entering a Therapy Plan or Complex Care Plan in Epic, please contact the Kootenai Help Desk at (208) 625-5555.

For Community Providers

Please contact Kootenai Infusion Services at (208) 625-6926 to request the order form for the medication you would like to prescribe for your patient. We will fax the appropriate order form directly to your office along with our Infusion Services Prior Authorization Checklist.

Once completed, please fax the order form and all required supporting documentation outlined in the checklist to Kootenai Infusion Services at (208) 625-6927 for processing.
