Kootenai Health thanks you for your partnership and we look forward to having your student(s) join us. If you have any questions about the information on this page or filling out the form below, please contact Kootenai Health’s Student Services department at (208) 625-6078 or at [email protected].
As we mentioned before, students must meet all Kootenai eligibility requirements before entering the hospital. Our affiliation agreement with your academic institution states that you must keep the following records on file at your academic institution for the student(s):
- A license (if applicable to your health profession)
- Immunization records that include:
- A Tuberculosis test within the last 12 months,
- 2 MMR vaccinations or titer
- Varicella vaccination, titer, or history of disease
- A state or national criminal background check
- Flu vaccination if the educational experience occurs during the flu season
- COVID-19 Vaccination(s)—must be Pfizer, Moderna, or Janssen
Kootenai asks that program faculty provide Student Services with a list of student names and their date of flu vaccination. This document must be provided prior to October 1st via a word or excel document to [email protected]
Even though these records are kept at your institution, Kootenai Health reserves the right to audit for them at any time during the next three years. If you would like to read through the details of the affiliation agreement we have on file with your program, please call the Student Services department (208) 625-6078. Thanks.
Important: Students placed in Kootenai Health’s Adult and Youth Acute Behavioral Health departments, chemical Dependency, Child of Adult Day Care Centers, or the Crisis Center must complete an Idaho Health and Welfare fingerprinted criminal background check. Idaho requires that anyone (including students) spending time with these at risk patient populations complete this requirement prior to beginning time at these facilities. This process takes 4-5 weeks for the state of Idaho to process. Get started by reading the Student Fingerprinting instructions.
